Apparently it is a Vox Treble Booster and a Maestro FZ-1b 4 transistor Fuzz. 

The layout is volume and blend with the 3 position toggle switches adding in the fixed treble boost. One for the treble side and one for the Fuzz side.

CONCLUSION __

Well ...Hey! The vintage casing is dead cool.  

I find this pedal ...questionable?
